Features of the results of ultrasound biomicroscopy in pseudoexfoliation glaucoma in combination with cataracts

Purpose:
Purpose is to study the features of the structures of the anterior segment of the eye in pseudoexfoliation glaucoma in combination with cataracts according to ultrasound biomicroscopy.
Setting:
Kazakh Eye Research Institute, Almaty city, Kazakhstan
Methods:
Ultrasound biomicroscopy was performed in 40 patients (80 eyes): 20 patients (40 eyes) of them were patients with pseudoexfoliation glaucoma combined with cataract; 20 patients (40 eyes) are with glaucoma without pseudoexfoliations. Ultrasound biomicroscopy was performed on a VuMaxII device (Sonomed, USA) with an ultrasonic frequency of 35 and 50 MHz, a scan depth of 5 mm, a resolution of 50 �Î�¼m. In both groups, the presence of pseudoexfoliations, the state of the zonule of Zinn, the width and reflectivity of the supraciliary space in the coronary part of the ciliary body at 1 mm from the scleral spur.
Results:
In patients with pseudoexfoliation glaucoma, zonule of Zinn was stretched (0.53 mm in comparison with 0.31 mm for glaucoma without pseudoexfoliations). The lens subluxation, according to ultrasound biomicroscopy, was detected 1.5 times more often than with biomicroscopy in patients of both groups. In the zone of the ciliary body, a symptom of rarefaction and decrease in the reflectivity of the flat part of the ciliary body was revealed, more pronounced with pseudoexfoliation glaucoma. The symptom is accompanied by an expansion of the supraciliary space, more pronounced with pseudoexfoliation glaucoma than with glaucoma without pseudoexfoliations (0.12-0.13 mm and 0.06-0.07 mm, respectively).
Conclusions:
The degree of pseudoexfoliation syndrome in glaucoma correlated with the degree of expansion of the supraciliary space. The revealed features of the results of ultrasound biomicroscopy in pseudoexfoliation glaucoma substantiate the conduct of deep sclerectomy for the purpose of draining the supraciliary space during the hypotensive and combined interventions for the prevention of postoperative choroidal detachment.
