Purpose:
Traumatic cataract is a challenge for the surgeon ophthalmologist, because frequently there are associated lesions (corneal or scleral, vitreo-retinal) and the integrity of the capsular bag is often compromised. Choice and implantation of the IOL (intraocular lens) become difficult in these situations. The visual outcome is often unpredictable and it is not only the lens that decides visual outcome. Multifocal IOLs are a rare choice in this cases.We describe a case in which the implant of a multifocal IOL after traumatic cataract was possible.

Emergency Eye Hospital, Bucharest
Methods:
We present the case of a 20 years old male patient with a corneal perforation and a metallic intraocular foreign body in his left eye; he also developed a traumatic cataract.Extraction of the intraocular foreign body is performed under local anesthesia using magnet. Subsequent delayed cataract surgery (after 2 weeks) is performed and a multifocal IOL is implanted.
Results:
Cataract surgery with cortex aspiration and implantation of a multifocal IOL (AcrySof IQ ReSTOR �,5D) went without intraoperative complications. After surgery the visual acuity without correction was 1 for both near and far vision.
Conclusions:
The implantation of a premium IOL should not be excluded in traumatic cataracts (even after an open-globe injury) when an accurate biometry is possible and a good centration in-the -bag is anticipated. The best approach for these cases for optimal visual outcome is a multistep procedure (whereever possible) with delayed cataract surgery.
