A clinical case of defect formation in Descemet's membrane of the cornea after femtosecond-assisted cataract phacoemulsification

Abstract Details

Purpose:

The clinical currency of corneal membrane defeat at extensive defect of Descemet membrane with following defect closing without surgical intervention.

Setting:

Sv. Fyodorov Eye Microsurgery Complex, Krasnodar Branch, Russia

Methods:

The cells condition of posterior epithelium corneal membrane and Descemet membrane were studied using endothelial microscopy, optical coherent tomography, algorithm of modern diagnostics and conservative treatment was applied.

Results:

Patient 68 years, male. After cataract phacoemulsification with femtosecond assistance in OD on 1 day II stage corneal oedema was ophthalmoscoped, on 2 day after performed dehydrative, anti-inflammatory and desensibilizing therapy in the center of cornea membrane a Descemet membrane defect 5.00mm in diameter was revealed. On discharge visometry OD 0.3 (uncorrected); tonometry according to Maklakov 21 mmHg; posterior epithelium cells are not determined. In 4 months after treatment visometry OD 1.0, tonometry OD 16mmHg; PEC 679(optical zone), 1263(paraoptical zone), cells are of one size of hexagonal form and located in the right rows.

Conclusions:

The applied tactics of conservative patient management allowed to achieve high VA, full coating of Descemet membrane defect with corneal posterior epithelium cells without surgical intervention.
