IOL calculation in patients with radial keratotomy

Abstract Details

Purpose:

Method of radial keratomy as treatment of different kinds of ametropy was proposed in 1974. It was the most popular keratorefractive operation untill era of eximer laser began. There are millions people all over the world with radial keratomy. More and more of them face with cataract development. IOL calculation in these cases is difficult, becouse the standart formuls give us a big mistake in the hyperopia side. The aim of our work is to make algoritm of IOL calculation in patients with radial keratomy.

Setting:

Clinic VISIOBOOD, Kyiv, Ukraine

Methods:

We performed our investigation on 13th patients (22 eyes) with radial keratomy in age 56-69 years. Phacoemulsification with IOL implantation was perfomed for all of them. Preoperatively all patients had standart examination: visometry, refractometry, keratorefractometry, optical biometry, ultrasaund biometry. If it was possible we used data of patient�â�€�™s keratometry before radial keratotomy.

Results:

IOL calculation in patients with data of keratometry before radial keratomy was easy. We used Shammas-PL formula. If data of preoperative keratometry were absent we assessed subjective refraction after keratotomy before cataract development, because it can change optical power of the lens. If patient after keratotomy had had emetropy, we implanted artificial lens of the power 23,0-24,0 diopters. Ametropy we calculated according to the one diopter of the power of artificial lens equals to 1,25 diopters of ametropy. In the postoperative period all patients had refraction from �,5 to -1,25 diopters. It was according to patients wait and operative plans.

Conclusions:

Calculation of the power of IOL in patients with radial keratotomy using standard formulas leads to mistake in the side of hyperopia. If the patient has the data of the keratometry before keratorefractive operation, it is necessary to use the Shammas-PL formula. If there are not these data the most exact calculation of the IOL power is the method of the subjective refraction assessment.
