Management of subluxated lens in Marfan syndrome

Abstract Details

Purpose:

To describe a simplified bag preservation surgical technique, and retrospective clinical report of the subluxated lens removal in patients with Marfan syndrome.

Setting:

Kazakh Eye Research Institute

Methods:

Retrospectively from 2012 to 2015, 18 patients with a clinical diagnosis of Marfan syndrome and clinically significant lens subluxation were reviewed. Patients underwent capsular bag safe lens extraction with one side IOL fixation. The pre- and postoperative best-corrected visual acuity, biometric measurements, intraocular pressure, and incidence of surgery-related complications were reviewed.

Results:

A total of 30 eyes of 18 patients were included. Mean postoperative follow-up was 1.2 �Â�± 0.7 years (range, 1-5 years). Average age at surgery was 10.2 �Â�± 9.2 years (range, 3-27 years), with 14 patients (28 eyes) �â�‰�¤ 18 years of age. The average preoperative best-corrected visual acuity was 20/25. Two eyes did not show any improvement and remained with the same visual acuity as before the surgery due to ablyopia. In one eye developed a retinal detachment following periferal retinal tear. No other intra- or postoperative complications were reported.

Conclusions:

Bag preservation surgery is safe and provides a consistent visual outcome in patients with lens subluxation secondary to Marfan syndrome. This is especially important in pediatric patients, because this approach is more physiological and has less potential postoperative complications.
