Evaluation of corneal changes and clinical results after coaxial torsional phacoemulsification in eyes with soft to hard nuclear density cataract

Abstract Details

Purpose:

To evaluate the corneal changes and clinical results of torsional phacoemulsfication in eyes with soft to hard nuclear density cataract.

Setting:

Department of Ophthalmology, School of Medicine, Namik Kemal University, Tekirdag, Turkey.

Methods:

In this study, 120 patients were classified according to Lens Opacity Classification System (LOCS) III and divided into six groups for coaxial torsional phacoemulsification and intraocular lens implantation.The main outcomes included corrected distance visual acuity (CDVA), central corneal thickness (CCT), central endothelial cell density (ECD), and clear corneal incision (CCI) morphology. Postoperative follow-up was at 1 day, 1 week, and 1 and 3 months.

Results:

Each group consisted 20 eyes of 20 patients. Changes of CCT was significantly higher in high density nuclear cataract group than the others at postoperative one day and one week (P < .01). At three months, the mean CDVA for each group was 0.04 logMAR and the mean CCT turned back to the preoperative level (P = .70 and P = .31, respectively). Endothelial cell density was significantly higher in high group than low group (P < .01). Morphological changes in the CCI were most marked in the high group at postoperative one day and one week (P < .05).

Conclusions:

Although the torsional phacoemulsification method saves the energy, endothelial cell loss after cataract surgery is most prominent in eyes with high density nuclear cataract.
