Predicted postoperative outcomes for cataract surgery after myopic keratorefractive surgery using the Pentacam Holladay EKR module

Abstract Details

Purpose:

To assess the predicted accuracy of the Holladay equivalent keratometry readings (EKR) module on the Pentacam scheimpflug camera system, in pateints who have previously undergone myopic laser keratorefractive surgery.

Setting:

Royal Hallamshire Hospital, Sheffield Teaching Hospitals NHS trust, Sheffield, United Kingdom

Methods:

Retrospective review of all cases on the electronic medical record who had undergone cataract surgery after laser myopic keratorefractive surgery and had a pre-operative Pentacam examination as of May 2015. The Holladay EKR values were applied by back calculation using SRK/T, from the actual post-operative refraction, to give a predicted post-operative refraction (PPOR) for the EKR. The deviation from the PPOR was compared with that actually achieved using the history method.

Results:

18 eyes in 14 patients underwent surgery between March 2014 and March 2015, fulfilling the above criteria. EKR PPOR had a mean absolute error of 0.94 dioptres (D) (range 0.06D to 2.50D) compared with the actual PPOR mean absolute error of 0.82D (range 0.02D to 2.06D). The EKR method underestimate the corneal power which would equate to a -0.23D shift in refractive outcome compared with the history method (p=0.03).

Conclusions:

Retrospectively, we found the Holladay EKR Pentacam module to underestimate the actual corneal power in this patient group. It produced greater error than the history method in post-laser refractive surgery patients undergoing cataract surgery.
