Abstract Details
Purpose:
Incidence of Cystoid macular oedema following apparently uneventful cataract surgery is well known.Inflammatory mediators are supposed to cause this.It causes a lot of post operative morbidity and is frustrating both to the surgeon and the patient.so if preservative free triamcinolone acitonide is injected into the anterior chamber routinely after uneventful cataract surgery or phacoemulsification the patient enjoys manifold advantages.
Setting:
The study was conducted at The Disha Eye Hospitals Sheoraphuli located in semi urban setting in Hooghly district of the state of west Bengal in India.
Methods:
All routine phacoemulsification patients were prospectively enrolled for the study. Central macular thickness by spectral domain OCT was recorded preoperatively.Exclusions included-complicated cases,Diabetics,intraoperative use of preservative free Adrenalin ,surgical time more than 7 minutes,PCR, and any other condition which involves excess maneuver in the anterior chamber and iris trauma.1mg of preservative free triamcinolone acetonide was routinely injected in the anterior chamber at the end of uneventful phacoemulsification with IOL under topical anesthesia.The patients are evaluated on days 1,7and 60days for central macular thickness by spectral domain OCT.The difference in macular thickness is noted.
Results:
The incidence of macular oedema after uncomplicated cataract surgery ranges from 1 to 41%depending on the definition and method of detecting CME. The incidence of CME with reduced vision measured by OCT is up to 14%.The incidence of cme in this study is 3.36%.This is profoundly significant.
Conclusions:
In spite of all the different advances in the field of cataract surgery pseudophakic CME remains a very common cause of reduced vision following uncomplicated cataract surgery.Oct was chosen to detect CME as this method has the highest sensitivity to detect CME.Preoperative control CMT was compared with the post operative CME at 6 weeks after operation.The incidence of CME was remarkably less(p<0.01) in our study as the pharmacological agent Triamcinolone in the anterior chamber has exerted sustained anti-inflammatory effect.
