Clinical results of toric intraocular lens implantation after laser-assisted cataract surgery versus conventional phacoemulsification

Abstract Details

Purpose:

To evaluate the visual and refractive outcomes of toric intraocular lens (IOL) implantation in eyes with corneal astigmatism after laser-assisted cataract surgery versus conventional phacoemulsification.

Setting:

Moscow, Eye Center �â�€�œEast Sight Recovery�â�€�.

Methods:

Patients with cataracts and corneal astigmatism from 1.5 to 4.50 diopters (D) were included in this study. 29 eyes of 23 patients underwent femtosecond laser-assisted phacoemulsification (Victus, USA) with AcrySof Toric IOL implantation . 28 eyes of 18 patients underwent conventional phacoemulsification with implantation of the AcrySof Toric IOL and En Vista Toric IOL. Preoperative examinations included optical biometry, Haigis IOL calculation, keratometry, corneal topography and objective and subjective refractions. Femtosecond laser-assisted stage involved anterior capsulotomy, lens fragmentation and paracentesis. Postoperatively, refractive outcomes, the mean residual refractive astigmatism, uncorrected visual acuity were assessed. The follow-up period was 9-18 months.

Results:

All patients were satisfied after surgery. The mean residual refractive astigmatism was 1.0 D �Â�±0.42 D in femtosecond laser-assisted group and 1.56 D �Â�±1.14 D in standard phacoemulsification group postoperatively The mean surgically induced astigmatism was 0.025 D �Â�± 0.07 in femtosecond laser-assisted group and 0.2 D �Â�± 0.25 D in conventional phacoemulsification group. Uncorrected distance visual acuity 20/25 or better was achieved in 78 % in both groups.

Conclusions:

The implantation of toric IOL in cataract patients with astigmatism allows to achieve a high level of visual acuity and patient satisfaction. Residual refractive astigmatism and surgically induced astigmatism were less in femto group versus conventional phacoemulsification.
