ORA was right! A case after radial keratotomy

Purpose:
To present the case of a patient with history of radial keratotomy who underwent phacoemulsification and ended with an unintended myopic refractive error

Case report
Results:
A patient with past history of radial keratotomy underwent phacoemulsification in her left eye. Double-K (using preoperative standard keratometry of 43,86) with Holladay 1 formula, according to the ASCRS calculator indicated �,30 D of power of the intraocular lens to be implanted. Barrett True K (ASCRS calculator) indicated 24,57 D. ORA intraoperative aberrometer recommended a power of �,00 D. Manual intraoperative aphakic refraction indicated �,00 D. Finally the surgeon implanted �,00 D. The first day after the surgery spherical equivalent was �,75 D, but one month later it was -2,125 D.
Conclusions:
ORA intraoperative aberrometer and Barrett True K method had significantly lower prediction errors than Double-K ẉ敩횲 1 and manual intraoperative aphakic refraction in one eye with past history of radial keratotomy. The early hyperopic shift after cataract surgery has to be taking into account by the surgeons to warn the patients. In this case, fortunately, the patient was satisfied with the unplanned monovision in which she ended.
