Comparison of corneal endothelial changes using two different phacoemulsification machines

Abstract Details

Purpose:

To compare the effect of 2 different phacoemulsification platforms, INFINITI Vision System (Alcon Laboratories, Inc., USA) against the more recent CENTURION Vision System (Alcon Laboratories, Inc., USA) in terms of changes in corneal thickness and endothelial cell count.

Setting:

Watany Research and Development Center (WRDC), Al Watany Eye Hospital, Cairo, Egypt

Methods:

Prospective non randomized interventional case series, including 25 patients,in the period between January and May 2016. I this study, one eye was operated by CENTURION Vision System and the 2nd was operated with INFINITI Vision System. Ultrasound pachymetry and specular microscopy were done, secondary outcomes included: UAVA, BCVA, machine parameters, CDE, Phaco- Time, Aspiration and Fluids.Follow ups were on the 3rd day and 3 months post-operative. Study was approved by the local ethical committee under Helsinki protocols.

Results:

Patients were 64% Females, mean age 61.8 (40-75), cataract grading ( 0=CLE and 4=grade 4 nuclear cataract); mean cataract grade in CENTURION was 2.16, in INFINITI group was 1.9, Mean CDE in CENTURION was 8.08(�4.8) in INFINITI was 11.72(�6.93), values for surgery time, fluidics and aspiration parameters were statistically non significant higher in CENTURION than INFINITI. CENTURION group showed better post-operative improvement in both the UCVA and BCVA. CENTURION showed slightly less increase in corneal thickness and better recovery to baseline. Also CENTURION showed slightly less better safety for the corneal endothelial.

Conclusions:

In spite of the CENTURION group having higher mean cataract grading, surgery time, fluidics and aspiration, this group demonstrated better results in turns of visual outcomes, corneal thickness and endothelial safety, although statistically insignificant, it shows the effect of the CENTURION producing less CDE than INFINITI phacoemulsification system.
