Total corneal astigmatism (TCA) and equivalent keratometry reading (EKR) of multicolor LED topographer for the choice of intraocular spherical versus toric lens

Abstract Details

Purpose:

To evaluate the use of total corneal astigmatism (TCA) for the choice of spherical or toric intraocular lens (IOL) and to determine the reliability of equivalent keratometry reading (EKR) for the cylindrical power calculations of toric IOLs. The study also aims to evaluate the contribution of TCA in the postoperative refractive outcomes in order to determine if the data taken into account for calculating the toricity of IOL were correct.

Methods:

This Prospective longitudinal study included 17 eyes with cataract and preexisting astigmatism. Precizon spherical or toric IOLs (Ophtec BV, Groningen, The Netherlands) were implanted on the basis of TCA measured with Cassini ((i-Optics BV, The Hague, The Netherlands); with minimum of 1.25D TCA for the choice of toric IOL. The cylindrical power calculation was based on EKR obtained from Cassini . At 3 months postoperatively, patients were examined for objective refraction using Topcon KR-1�Â�® auto-refractometer and best subjective correction.

Results:

The mean age of the subjects was 71 years and with mean cataract grade was 2. Of the implanted lenses, 58.8% had spherical and 41.2% were implanted with toric lenses. At postoperative 3 months, mean uncorrected visual acuity was 0.1 and 0.2 for spherical and toric IOLs respectively (logMAR scale). Residual objective refraction for spherical IOLs was 0.21D (sphere) and 0.35D (cylinder) and; for toric IOL was �.75D (sphere) and -0.75D (cylinder). With subjective refraction, spherical IOLs had �.21D (sphere) and -0.57D (cylinder) and toric IOLs was 0 D (sphere) and 0.53D (cylinder) for toric IOLs.

Conclusions:

86% of the eyes implanted with spherical lenses had residual astigmatism of less than 0.5D and 100% of the toric IOL implanted eyes had residual astigmatism of less than 0.75D. Thus, TCA and EKR are effective in guiding the choice of spherical or toric IOLs and performing the cylindrical power calculation of toric IOLs.
