Advanced cataract surgery: laser-assisted cataract surgery with preoperative guidance diagnosis and intraoperative aberrometry in cases of cataract and corneal astigmatism with toric IOL implantation

Abstract Details

Purpose:

To analyze technologies such as femtosecond laser (LenSx), intraoperative aberrometry (ORA System) and preoperative guidance diagnosis (Verion) in cases of cataract and corneal astigmatism treated with toric IOL implantation.

Setting:

Department of Ophthalmology.'Lozano Blesa' University Clinical Hospital of Zaragoza, Spain.

Methods:

Twenty eyes with corneal astigmatism �â�‰�¥ 2D and cataract were studied. All cases had a dense cataract with a vision below 4/10. Preoperative variables are analyzed: pseudoexfoliative syndrome, narrow anterior chamber, previous iridotomies, previous keratoplasty, cataract hardness and pupillary size after dilatation; the intraoperative findings after the femtosecond laser: efficacy of capsulotomy, incision, maintenance of dilatation, difficulty in extracting the crystalline cortex. We also analyzed the postoperative clinical and refractive outcomes after IOL implantation with the intraoperative aberrometry (ORA System) and preoperative guidance diagnosis (Verion) assistance.

Results:

A complete capsulotomy was performed in all cases. In 6 cases the pupillary size was significantly reduced, coinciding with the cases of narrow chamber or the presence of intraoperative IFIS. After cataract extraction, ORA System indicates to us the power of the IOL that we must implant. Verion indicates the axis of implantation without needing to mark the cornea. After implantation, ORA System allows the exact adjustment of the axis. In all cases we achieved the correction of the spherical and astigmatic refractive error of the patient with a residual defect of less than 0.5 D.

Conclusions:

ORA System indicates the spherical and toric power of the IOL that we must implant. Verion indicates the axis of implantation of the toric IOL in the eyepiece of the microscope, without needing to mark the cornea. After implantation of the IOL, ORA System allows the exact adjustment of the axis for the neutralization of the corneal astigmatism and the verification of the permanence of the IOL in the correct position after the end of the surgery. Femtosecond laser (LenSx), intraoperative aberrometry (ORA System) and preoperative guidance diagnosis (Verion) improve refractive results in procedures involving toric IOL implantation.
