Outcome of management of posterior capsular rent during phacoemulsification

Abstract Details

Purpose:

To describe the outcome of management of posterior capsule rupture during phacoemulsification.

Setting:

Tertiary care eye hospital of eastern India

Methods:

In this retrospective study all cases of phacoemulsification with posterior capsular ruptures between 2012 and 2016 were analyzed to determine the best-corrected visual acuity at 3 months.They were additionally studied in relation to age, sex, type of cataract, preoperative risk factors, stage of surgery, vitreous loss, dropped nucleus, and other complications like endophthalmitis, retinal detachment, corneal decompensation, dislocated / decentered intraocular lens. Success was defined as postoperative visual outcome >/=20/40 at 3 months.

Results:

From 2012 to 2016, there were 87 capsule ruptures in 9736 phacoemulsification cases with a rate of 0.89%. Visual success was achieved in 90.8% cases. Ruptures most frequently occurred during irrigation and aspiration followed by phacoemulsification. Risk factors for poor outcomes included diabetes, dropped nuclei, and other complications like postoperative CME and others

Conclusions:

Appropriate management of posterior capsular ruptures during cataract surgery can lead to optimal postoperative outcome which is usually affected by diabetes, dropped nuclei, and postoperative retinal and IOL complications.
