Method of late IOL dislocation prophylaxis in pseudoexfoliation syndrome

Abstract Details
Purpose:
To develop a method of IOL fixation for prophylaxis of IOL subluxation in pseudoexfoliation syndrome. It includes implantation of a capsular tension ring into the capsular bag, IOL implantation with its suture fixation (polypropylene 10/0) to the iris in the inferior sector after pushing aside the anterior capsule.

Methods:
The study included 17 eyes of 15 patients (9 males, 6 females, aged from 61 to 90 years). All the eyes had grade I �â�� III pseudoexfoliation syndrome, with phacodenesis in 12 eyes. Preoperative visual acuity ranged from light perception to 0.7 (mean, 0.2 �Â�±0.2). In 11 eyes phacoemulsification was performed using a capsular tension ring incarcerated in paracentesis for additional fixation of the capsular bag. No vitreous loss was seen. Implanted and sutured IOLs included 10 three-piece, 6 single-piece and 1 toric lens.
Results:
Post-op follow- up made 12 to 50 months (mean, 26.9 �Â�±13.1 month). At the end of follow-up mean visual acuity was 0.65�Â�±0.32 (range, 0.05 to 1.0). No IOL dislocation was seen, but in 6 cases (35%) slight inferior shift of the suture with pupil stretching was marked which may indicate weakening of the superior portion of Zinn�â��s zonulae. We consider the method to be useful in patients with pseudoexfoliation syndrome including phacodenesis, inferior zonular defect no more than 180 degrees, IOL dislocation caused by pseudoexfoliation syndrome in the fellow eye.
Conclusions:
The suggested method of IOL fixation is simple, allows reliable fixation of IOL �� capsular ring �� capsular bag complex with a possibility to avoid late dislocations. Further follow-up and long-term analysis are required.
