Resistant pseudophakic cystoid macular edema

Abstract Details

Purpose:

To present a case of our patient with cystoid macular edema after an uneventful cataract surgery, resistant to any conservative treatment.

Setting:

IzakVisionCenter s.r.o., Nad Plážou 21/A, 97401, Banská Bystrica

Methods:

58-years old man with light hyperopia and incipient cataract underwent cataract surgery with multifocal IOL implantation. One month after the surgery cystoid macular edema occured in patient's right eye. The patient received all available conservative treatment of macular edema (topical use of NSAID drops, topical, intravenous and oral corticosteroids), intvavitreal corticoid (dexamethason), intravitreal anti- VEGF (bevacizumab) and at last pars plana vitrectomy.

Results:

Late diagnosis and frequent recurrences can cause damage of foveolar photoreceptors and permanent decrease of central visual acuity. In our case, only the pars plana vitrectomy ensured satisfactory result.

Conclusions:

Early diagnosis and consequent treatment of pseudophakic cystoid macular edema is mandatory. Risk of pseudophakic macular edema can be decreased by topical use of NSAID drops 1 day prior the cataract surgery and daily over the next 3 weeks postoperatively.
