The analysis of clinical-economic effectiveness of topical levofloxacin 0,5% and levofloxacin oral forms combination in antibacterial prophylaxis in cataract surgery

Abstract Details

Purpose:

The clinical-economic effectiveness assessment of preoperative antibiotic prophylaxis with systemic fluoroquinolones within cataract surgery.

Setting:

Primorsky center of Eye Microsurgery. Russia,Vladivostok

Methods:

9314 phacoemulsifications were made in the period of time from 2012 to 2014. 3087 patients (4662 eyes) were receiving topical Oftaquix drops four times daily preoperatively for 3 days and postoperatively for 10 days. 3015 patients (4652 eyes) were taking Levofloxacin 500mg daily for 5 days additionally

Results:

13 postoperative infectious complications were identified: in 12 eyes in first group and in 2 eyes �â�€�“ in second one. Relative risk (RR) was 0,08; 95% confidence interval (CI) was from 0.0108 to 0.6393, P = 0.0169; NNT index �â�€�” number needed to treat was 422.827 (95% �Ð�¡I = 1177.329 to 257.686 (Harm). The expenses on antibiotic prophylaxis were twice as high at the department 2, however expenses on the treatment of developed postoperative infectious complications, including examination, endovitreal surgery, intravitreal antibiotic injections and postoperative antibiotic therapy, were 12 times lower.

Conclusions:

Thus, results of research let conclude that systemic usage of fluoroquinolones preoperatively in cataract surgery is effective prophylaxis method in postoperative infectious complications prevention as in clinical well as in economic aspect
