Acute calcific bant keratopthy in a patient with renal osteodistrophy after phacoemulsification

Abstract Details

Purpose:

To present a case of acute calcific band keratopathy following an uneventful phacoemulsification and intraocular lens implantation

Setting:

Uludag University, Department of Ophthalmology, Bursa, Turkey

Methods:

A 77 years-old female referred to Department of Ophthalmology with complaints of tearing and blurred vision after cataract surgery at a private eye clinic. She had underwent an uneventful phacoemulsification and intraocular lens implantation for cortical cataract in her left eye. She had been receiving topical ofloxacin 0.3 % and dexamethasone 0.1 % for 3 weeks. She had chronic renal failure and had been also receiving calcitriol and calcium acetate for renal osteodistrophy.

Results:

The biomicroscopic examination revelaed grayish- whitish deposits on corneal surface in the interpalpebral area of her left eye. According to patient discharge report,there was no corneal deposits at the time of cataract diagnosis and the deposits were noted in 2 weeks time following the surgery. Chelation with Na EDTA and debridement was performed and bandage contact lens was placed. The epithelium healed in one week.

Conclusions:

It should be kept in mind that even minor surgical trauma to endothelium may trigger calcium deposition on corneal epithelium and Bowman layer in patients with chronic renal disease who may have impaired calcium and phosphate metabolism leading to metastatic calcification. The patients should be informed for these consequences before the surgery.
