Abstract Details

Purpose:

Ultrathin DSAEK has now become established as an effective method of visual rehabilitation in endothelial failure. In the majority of units in the UK the Moria system is the preferred method of automated dissection. The Gebauer system differs as it is fully automated, and ultrathin grafts may be achieved using a single pass. In this study we evaluated the results of our DSAEK surgery using the Gebauer microkeratome over a year period.

Setting:

The study was carried out at Queen Alexandra Hospital in Portsmouth, a large public hospital in the south of England.

Methods:

Patients operated on over a 4 year period with at least 6 months follow up were found using an electronic patient record, and paper notes were reviewed to collect data.

Results:

87 eyes were analysed, 10 of which were precut in the eyebank using the Gebauer system. Ultrathin grafts were achieved in 79% of cases. 90% of patients achieved a best corrected visual acuity of 6/12 or better, and 72% achieved 6/9 or better at 6 months. The graft detachment rate in grafts cut at our instituation was 19%, whilst those precut at the eyebank was 50%.

Conclusions:

The Gebauer sytem is capable of cutting ultrathin grafts in the majority of the time. It achieves good visual outcomes. The detachment rate of grafts is higher for material that has been precut prior to the day of surgery.
