Modified bean-shaped ring segments for suture fixation of the bag-in-the-lens intraocular implant

Abstract Details

Purpose:

To present a novel surgical technique for secondary stabilization of a Bag-in-the-lens intraocular lens (BIL IOL) using two modified bean-shaped ring segments in a case of severe zonular dehiscence associated with pseudoexfoliation.

Setting:

Department of Ophthalmology, Antwerp University Hospital, Wilrijk, Belgium

Methods:

A 77-year old female presented with a decreased best-corrected visual acuity (0,2) after she was treated 8 years ago for cataract with a BIL and capsule tension ring. Extensive zonular lysis resulted in luxation of the lens-bag complex and the patient was offered a second surgery to reinforce the suspension of the IOL. An additional anchoring element of the segment to the sclera was needed. This was achieved by modifying our previous described bean segments and adding an eyelet in the centre of the inner face of the outer semicircle in order to facilitate suturing it to the sclera.

Results:

The first modified bean-segment was anchored with a suture to the sclera in the area of maximum zonular dehiscence. The second segment was then implanted in the opposite sulcus area and both segments are placed in the BIL IOL interhaptic groove. The segments stabilized and centered the BIL IOL by creating an “artificial” zonule which provides the necessary extra support for the IOL. After surgery, the modified bean segment and the lens position significantly improved. Post operatively her BCVA had improved to 0,9.

Conclusions:

In patients with severe zonulopathy, stabilization and centration of an IOL can be technically challenging. An additional anchoring element to the bean segment facilitates segment suture to the sclera, ensuring better support and centration of the bag and of the BIL IOL.
