Outcome of combined topical treatment of cystoid macular edema using corticosteroids and non-steroidal anti-inflammatory eye drops, optical coherence tomography based study, Alsaim Eye Hospital, Sudan, Nov 2013 – Mar 2016

Abstract Details

Purpose:

This is a hospital based study aimed at determining the outcome of treatment of CME using combined topical treatment of diclofenac sodium 0.1% and corticosteroids, based on Optical coherence tomography “OCT” appearance.

Setting:

Alsaim Eye Hospital, Medani, Gezira state, Sudan Nov 2013 –Mar 2016

Methods:

Eligible patients were counseled before commencing the treatment, put on regular follow up. Twenty nine eyes received prednisolone acetate 1% and diclofenac sodium 0.1% eye drops, one patient received added oral prednisolone and one eye received dexamethasone and diclofenac sodium 0.1% eye drops. Follow up criteria: patient's complaints, visual acuity, intra-ocualr pressure and OCT. Analysis was done using Microsoft Excel 2010.

Results:

of the thirty two eyes; (88%) showed improvement; of them (56%) showed complete resolution of the cyst with reduction of the average OCT from 415 μm to 268 μm, in an average of 4.6 months, (13%) with cyst resolution, average post treatment OCT 314 μm in 2.7 months, and (19%) are still on follow up average OCT 387 μm and average duration is 1.5 months. (3%) developed elevated IOP, and (11.7%) considered as failed cases and shifted to other treatment.

Conclusions:

Inflammation is incriminated as one of the pathological mechanisms of CME, based upon this; many pharmacological anti-inflammatory agents are being tested. Although so far the results are promising; yet a little is certainly known about the long term effects, and until recently no single treatment is defined and proved to be suitable for all the forms of CME. This study provides a good evidence for an optimistic consideration of the efficiency of the combined topical treatment, and expands the horizons for larger controlled studies to be carried out.
