Abstract Details

Purpose:

To evaluate the efficacy and necessity of implantable Collamer lens (ICL) V4c with paired opposite clear corneal incisions for correction of compound myopic astigmatism.

Setting:

Qingdao Eye Hospital, Shandong Eye Institute, Shandong Academy of Medical Sciences, Qingdao, China

Methods:

Forty eyes of 20 patients with myopic astigmatism and 40 eyes of 20 emmetropic subjects were recruited. Preexisting corneal astigmatism was more than 1.50 diopters (D) in the myopic eyes. Paired opposite clear corneal incisions were made on the steep axis, one for implantation of ICL V4c and the other for astigmatic correction. Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), keratometry, ciliary body cysts, and optical quality were recorded.

Results:

With 3 months follow-up after ICL surgery, the mean postoperative logMAR UCVA (-0.05 ± 0.07) was significantly better than the mean preoperative logMAR BSCVA (0.03 ± 0.04, P<0.001). The mean surgically induced astigmatism was 1.35 ± 0.35 D (range, 0.93 to 2.09 D). For 3-mm and 5-mm pupils, no significant differences were found in total higher order aberrations and spherical-like aberrations between the ICL eyes and emmetropic eyes (P>0.05). There were 22 eyes (55%) presented ciliary body cysts in the myopic patients and most cysts were in the inferior (72.7%) and temporal (77.3%) sectors.

Conclusions:

ICL V4c with paired opposite clear corneal incisions to correct myopia with astigmatism is a useful way to keep away from larger cysts for the patients with astigmatism and cysts.
