Long-term 5-year safety and refractive outcomes of phakic intraocular lens implantation in patients with anterior chamber depth between 2.8 – 3.0 mm and superior to 3.0 mm

Abstract Details

Purpose:

To compare the long-term 5-year endothelial cell count (ECC) variation after phakic intraocular lens (pIOL) implantation to treat high myopia in patients with pre-operative anterior chamber depth (ACD) between 2.8 and 3.0 millimetres and superior to 3 millimetres. Analysis of the global visual and refractive outcomes is also performed.

Ophthalmology Department, Centro Hospitalar e Universitário de Coimbra, Coimbra, Portugal.

Methods:

Retrospective case-series study. High myopic patients submitted to primary pIOL implantation between January 2009 and December 2011 were reviewed. Pre and postoperative logMAR uncorrected visual acuity (UCVA), best-corrected visual acuity, manifest refraction and ECC were collected. Baseline ACD (endothelial to anterior lens surfaces) was measured with Orbscan IIz system. Cases were divided in two groups, according to preoperative ACD: group A – ACD between 2.8 and 3.0 millimetres; group B – ACD superior or equal to 3.0 millimetres.

Results:

A total of 131 eyes of 78 patients were included, 40.5% (n=53) in group A and 59.5% (n=78) in group B, with a mean follow-up of 5.58±0.46 years (5.00-7.14). Spherical equivalent decreased from -12.05±4.34 to -0.47±0.79 (p<0.001). Mean postoperative UCVA was 0.12±0.16 in group A and 0.08±0.17 in group B (p=0.161). Mean annual ECC loss was -83.55±61.18 in group A and -64.54±63.87 in group B (p=0.098), with an annual percentage loss of -2.86±1.86 in group A and -2.29±2.23 in group B (p=0.127). No pIOL was explanted due to ECC loss.

Conclusions:

The present study suggests a non-significant trend for long-term unfavourable safety and refractive outcomes profile in patients with shallower anterior chambers (2.8-3.0mm) prior to pIOL implantation.
