Management of anterior pole dysgenesis: two case reports

Abstract Details

Purpose:

To show the outcomes of management of two cases of anterior pole dysgenesis.

Setting:

Cornea and Ocular Surface Department of a Tertiary Referral Ophthalmologic Hospital (Málaga. Spain).

Methods:

Case 1: A 6-day-old girl was referred for bilateral corneal opacity. Diffuse corneal turbidity with iridocorneal adhesions was observed. The corneal diameter was 10mm in both eyes and the intraocular pressure (IOP) was 30mmHg with pachymetry values of 508 in right eye and 450 in left eye. Case 2: A 1-day-old boy due to corneal opacity in right eye. Total corneal opacity with iridocorneal adhesion in periphery was observed. In the left eye the anterior pole was normal. The corneal diameter was 12mm. The IOP was 28mmHg with pachymetry values of 624.

Results:

Case 1: After intensive topical treatment to reduce IOP values, trabeculoplasty surgery was performed. Once the pressure is controlled, proceeds to perform the keratoplasty. Case 2: After intensive topical treatment to reduce IOP, a conservative treatment was advocated.

Conclusions:

The malformations of the anterior segment are alterations in the normal pattern of development and may involve exclusively a structure such as the cornea or involve all structures of the anterior chamber. In cases of dysgenesis of the anterior pole a correct differential diagnosis must be made through the data of biomicroscopy, corneal diameter, intraocular pressure and pachymetry, and not forgetting the possibility of associated glaucoma.
