A case series of defective toric IOLs with no effects on visual outcomes

Abstract Details

Purpose:

To report a case series of toric intraocular lenses (IOLs) (Acrysof Toric, Alcon) with noted paracentral defects after implantation and study their effects on visual outcomes.

Setting:

Saint George Hospital – University Medical Center, Beirut, Lebanon.

Methods:

Seven toric IOLs implanted in 7 eyes with cataract and corneal astigmatism of more than 1.00D during regular uncomplicated phacoemulsification cataract surgery were noted to have paracentral linear material defects after implantation. Defects resembled microbubbles inside the lens material with smooth optical surface and were all paracentral. All patients were followed up at one day, one week and one month postoperatively. Slitlamp photos were taken and uncorrected visual acuity (UCVA) was documented.

Results:

Since the defects were in the paracentral area of the lens optics, decision was taken not to remove the IOL. At all follow up intervals, UCVA was found to be unaffected ranging from 0.1 to 0 logMAR and no change in shape or size of the defects was observed (as documented by slitlamp photos). None of the seven patients reported any glare or visual disturbance postoperatively.

Conclusions:

To our knowledge, this is the first report of defective toric IOLs with no consecutive effects on vision. Further studies need to be conducted to find out the possible causes and prevent further defects that might affect vision if occurring centrally and pose technical challenges in IOL exchange.
