Abstract Details

Purpose:

To report the problems with diagnosis and care of a patient with ocular siderosis

Setting:

Department of Ophthalmology, Poznan University of Medical Sciences, Poznan-Poland.

Methods:

We describe a case of a 52 -year- old man who had been recognized with a foreign intraocular body in magnetic head resonance 16 months after ocular trauma. Ophthalmic examination revealed iron deposits in cornea, heterochromic iris, cataract, macular cotton wood spots and optic disc gliosis. We observed specific changes in electroretinographical (ERG ) examination - b wave reduced more than 75% of the fellow eye. We performed combined surgery : phacoemulsification with artificial lens implantation and 23G pars plana vitrectomy with foreign body removal .

Results:

One week after the surgery the visual acuity improved from 0,6 to 0,9 but 4 weeks later we observed vision deterioration to 0,6 because macular edema with retinal ischemic changes.

Conclusions:

Visual acuity after removing of iron foreign intraocular body and cataract depends on the changes in the retina. ERG can be a good prognostic tool in patient with ocular siderosis
