Description of Fuchs endothelial dystrophy in HD-optical coherence tomography with correlations to visual acuity

Abstract Details

Purpose:

To describe corneal morphology in Fuchs‘ endothelial dystrophy high-definition optical coherence tomography imaging and to correlate those changes to visual acuity.

Setting:

Retrospective analysis of 60 eyes suffering from Fuchs’ endothelial dystrophy and a healthy control group using HD-OCT corneal images and visual acuity.

Methods:

HD-OCT corneal images were taken during daily clinical routine using the Zeiss Cirrus 5000 platform. Thickness of corneal epithelium, the Bowman’s layer, the stroma and the Descemet‘s membrane (including endothelium) was determined with the built in software. Grey value color threshold of the OCT images was measured using Image J. Best corrected visual acuity was presented in logMAR. All observed values were described as mean and standard deviation. Statistical comparison and correlation was achieved using SPSS. All patients with any other vision limiting disease except Fuchs’ endothelial dystrophy were excluded regarding visual acuity analysis.

Results:

Total corneal thickness (p < 0.001), stromal thickness (p = 0.002) and Descemet’s membrane thickness (including endothelium) (p < 0.001), varied significantly in comparison to the healthy controls. Bullae and irregular wavelike thickness changes were observed in Fuchs endothelial dystrophy patients. Corneal thickness correlated well with visual acuity in patients with Fuchs’ endothelial dystrophy. Furthermore, grey value colour threshold of the OCT images correlated with visual acuity and a higher amount of hyperfluorescence was seen in Fuchs patients when compared to control eyes.

Conclusions:

HD-OCT imaging might become a helpful tool to determine morphological changes in patients with Fuchs’ endothelial dystrophy. Several of the measured parameters correlated with a reduction of visual acuity. In future, this might help to determine the impact of corneal morphological changes on visual acuity especially if there are multiple eye pathologies present in the same eye.
