Visual acuity prognostication following cataract phacoemulsification in patients with high myopia

Abstract Details

Purpose:

To study the correlation between the probability of visual acuity improvement to a certain level followed cataract phacoemulsification with intraocular lens implantation and original functional anatomical parameters in high myopia.

Setting:

The Academician S.N. Fyodorov FSАI IRTC “Eye Microsurgery” Tambov branch, Tambov, Russia

Methods:

The study included 93 patients (103 eyes) with cataract and high myopia. The basic methods were ultrasound biomicroscopy of the retinal periphery, ultrasound B-scan of the vitreous body and spectral-domain optical coherence tomography of the macular zone preoperatively, in a month, in 6-8 months. Patients’ mean age was 65.04±1.16 years old, spherical equivalent refraction was 13.63±0.59 D; best-corrected visual acuity (BCVA) was 0.24±0.02 preoperatively. The length of the ocular anterior posterior axis was 28.17±0.23 mm.

Results:

Correlation analysis showed that the postoperative VA depended directly on the preoperative BCVA, foveal adhesion of the posterior hyaloid membrane and depended inversely on the magnitude of the anterior posterior axis, spherical equivalent refraction, the height of the posterior vitreous detachment, retinoschisis in the inferior-interior segment, the height of the epiretinal membrane, disorganization of the retinal pigment epithelium. Models predicting the probability of achieving a certain level of visual acuity in 1 and 6 months followed phacoemulsification were constructed by the binary logistic regression method. The proportion of correct forecasts varied from 75.86%-95.35% depending on the specified VA level.

Conclusions:

Visual acuity followed phacoemulsification correlates with the original functional anatomical parameters in high myopia with high statistical significance. The obtained data allow determining with high accuracy the patients with poor and high visual prognosis after cataract surgery that will help to carry out medical and preventive measures aimed at stabilization of visual functions in these patients postoperatively.
