Recovery of corneal clarity after DMEK followed by re-bubbling versus uneventful DMEK

Abstract Details

Purpose:

To evaluate corneal densitometry (CD) after Descemet membrane endothelial keratoplasty (DMEK) followed by Rebubbling versus uneventful DMEK.

Setting:

Department of Ophthalmology, Philipps-University, Marburg, Germany

Methods:

Medical records of 130 consecutive DMEK cases (118 patients) were reviewed. After excluding cases with prior corneal surgery, primary or secondary graft failure, two groups were formed. Group 1 (n=32): eyes undergoing DMEK with subsequent Rebubbling due to postoperative graft detachment and group 2 (n=80): eyes undergoing DMEK without further procedures. Scheimpflug CD of the optically relevant (0-2mm and 2-6mm) and peripheral (6-10mm) zone, endothelial cell density (ECD), central corneal thickness (CCT) and corneal volume (CV) were evaluated preoperatively and at 3, 6, and 12 months postoperatively.

Results:

Central CD (zones 0-2mm and 2-6mm) decreased significantly in both groups (P≤0.011). CD at the periphery(zone 6-10mm) decreased significantly in group 1, however not in group 2 (P=0.222). CD was significantly higher in group 1 at 3 months (zones 2-6 and 6-10mm) (P=0.037 and 0.035, respectively). ECD was not significantly higher in group 1 (P≥0.073). Finally, CCT and CV did not show a significant difference between the two groups.

Conclusions:

The presented data suggest that Rebubbling allows corneal clearance to similar levels as uncomplicated DMEK while causing similar endothelial cell loss.
