Corneal densitometry after DMEK: midterm results and correlations with endothelial cell density, central corneal thickness and corneal volume

Purpose:

To analyse the course of corneal densitometry (CD) after Descemet membrane endothelial keratoplasty (DMEK)

Setting:

Department of Ophthalmology, Philipps University, Marburg, Germany

Methods:

Medical records of 118 consecutive patients (130 eyes) who underwent DMEK were reviewed. Eighty pseudophakic eyes without prior corneal surgery, intra- or postoperative complications were included (group1). Scheimpflug CD of the optically relevant (0-2mm and 2-6mm) and mid-peripheral (6-10mm) zone, endothelial cell density (ECD), central corneal thickness (CCT) and corneal volume (CV) were evaluated preoperatively and at 3, 6, 12 and 24 months postoperatively. Correlations between CD, ECD, CCT, and CV at each examination time point were calculated. The 24-month results were compared to age-matched pseudophakic eyes (group2; n=20), as well as to phakic eyes of young subjects (group3; n=30).

Results:

CD at 0-2mm was 35.4±11.0GSU preoperatively and 19.5±3.0 at 24 months (P=0.012). Similarly, CD at 2-6mm was 28.8±8.4GSU preoperatively and 19.5±3.4 at 24 months (P=0.034). ECD fell from 2444±187cells/mm2 to 1332±431 after 24 months (P<0.001). CCT was reduced from 706±120µm preoperatively to 535±38 at 24 months (P<0.001). CV at 0-10mm zone was 66.7±8.6mm3 preoperatively and 61.8±3.7 at 24 months (P=0.032). CD at 24 months after DMEK was similar to that of healthy pseudophakic eyes (0-2mm, P=0.087; 2-6mm, P=0.208; 6-10mm, P=0.179).

Conclusions:

CD shows a continuous decrease over the first 24 postoperative months after uneventful DMEK reaching similar levels to corneas of age-matched pseudophakic controls. The present results suggest an optimal restoration of the corneal clarity achieved with DMEK.
