Purpose:
To evaluate and compare the corneal surface parameters, keratoconus indices, thickness profile data, and data from enhanced elevation maps of early keratoconic and thin non keratoconic corneas with the Pentacam Scheimpflug corneal tomography and to study the usefulness of different parameters to differentiate KC from topographically normal thin corneas(TC).
Setting:
National Ophthalmology Center named after acad. Z.Aliyeva, Azerbaijan.
Methods:
The study included 49 eyes with early stage of keratoconus (KC) and 49 healthy eyes with minimal pachymetry ≤ 499 µm, a mean age of 21.19±2.97 and 21.5±2.95 years, respectively. Among the keratoconus patients 22 eyes were included in subclinical KC group and 27 eyes in mild KC group. The Mann-Whitney U test was used to compare the data between the groups and the ability of the parameters to distinguish KC from non keratoconic thin cornea (TC) was assessed by receiver operating characteristic (ROC) curve analysis
Results:
No significant differences were found in terms of such as mean keratometry, astigmatism, corneal volume between study groups. But the majority of various elevation, thickness and topometric indices had sufficient strength (AUROCs > 0.90) to differentiate mild KC from TC. Pachymetric progression index maximum (PPI max), percentage thickness increase (PTI) 2, posterior elevation, and D values show the excellent AUROCs with 100% sensitivity and 100% specificity in discrimination TC from mild KC eyes. However among all parameters studied the anterior elevation (0.938) and keratoconus index (0.904) show the excellent AUROCs to differentiate subclinical KC from TC.
Conclusions:
This study showed that several indices derived from Pentacam measurements, including keratoconus indices, data from enhanced elevation and thickness profile maps are helpful in morphological differentiating eyes with keratoconus from thin corneas.
