Purpose:
Dry eye is the most frequent cause of ocular discomfort In TAO and role of T cell dependent inflammation has been implicated in the pathogenesis. The purpose of our study is to prove that T cell dependent inflammation contributes to the dry eyes in patients with grave disease prior to proptosis and lid lag, which has been described as main cause of dry eyes in TED.
Impression cytology is standard technique to study squamous metaplasia of conjunctiva and goblet cells loss in ocular surface disorder

Methods:
Study Design: Prospective Interventional/ observational Study
Study Site: Mahatma Gandhi Hospital, Jaipur
Study Period: 12 months
Inclusion criteria: 25 newly diagnosed patients of Graves disease ( Group 1) and 25 healthy subjects (group 2) were selected
Both the groups examined for visual acuity, lid lag, palpebral fissure width, Proptosis, Ocular motility and fundus
Schirmers test (without topical anaesthesia) TBUT, OSDI were performed
Impression cytology was done from the temporal interpalpebral bulbar conjunctiva and graded according to Nelson grading
Results:
Patient with hyperthyroidism were more symptomatic for dry eyes as compared to control.
Mean OSDI score in the patient group was 47.2+-12 as compared to 26+-10 in control group.
Mean Schirmer test score was 12.62+- 5.26 in cases as compared to 22.64+-5.6 in control group.
Mean Tear Film Break up Time (TBUT) was 7.28+-3.1 second in cases as compared to 10.64+-1.6 seconds in control.
Impression Cytology: grave’s patients showed significant Ocular Surface damage in which 78% had grade 2-3 squamous metaplasia in temporal inter-palpebral conjunctiva in comparison to control.
Conclusions:
Proptosis and Lid retraction are not only the pathological cause for dry eyes. Ocular surface inflammation characterised by metaplastic changes also contribute to the pathogenesis of dry eyes in patients of hyperthyroidism.
