Purpose:
To report two cases of bacterial keratitis in patients who underwent collagen cross-linking epithelial-off ( CXL) procedure for keratoconus with application of soft bandage contact lens.

Results:
First case was a 37 year old male presented with abscesses and cyclitis in the treated eye two days after surgery. Bacterial's colture was negative in the infected eye(he was using topical antibiotic drops when corneal scraping was performed)and positive in the other eye for Staphilococcus aureus.Second case was a 33 years old male whit a similar infection 3 days after CXL performed in the left eye:bacterial's colture resulted negative in both eyes.Patiens were treated with Moxifloxacina and Netilmicin eye drops every hour and oral Ciprofloxacin.Afterwards antibiotic therapy was modified starting Ceftazidime and Vancomycin fortified eye drops for 30 days.
Conclusions:
Both infective keratitis was succesfully treated with topical therapy, but at 3 month follow-up postoperatively slit lamp esamination showed completely quiet eye with a central macular grade corneal opacity with distinct borders which compromises visual acuity.
CXL has been widely used in treatment of corneal ectatic conditions and complications such as infective keratitis are uncommon. We present two cases of severe microbial keratitis although the exact mechanism of pathogenesis is unclear. Nonetheless, patients should be well-informed of such potentially complication.
