Abstract Details
Purpose:
To evaluate the refractive and clinical outcomes of accelerated corneal collagen cross-linking (aCXL) using the protocol 9mW/cm2 in 10 minutes performed in eyes with progressive keratoconus at 1year follow-up.
Setting:
Retrospective study in Gemini Eye Cinic, Prague, Czech Republic
Methods:
23 eyes with progressive keratoconus underwent epi-off aCXL using the riboflavin 0.1% solution containing 20% dextran every 2 minutes for 30 minutes before, and during the 10 minutes of UVA irradiation (9 mW/cm2) by KXL Avedro crosslinking system. The corneal epithelium healing, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), objective and manifest refraction changes, Pentacam corneal tomography, endothelial cell density (ECD) were measured before aCXL and 12 months after surgery and these were compared with preoperative findings. One month postoperatively corneal stromal demarcation line depth was measured in all patients using anterior segment optical coherence tomography.
Results:
The corneal epithelium healing lasted an average of 2.75 days. There was no endothelial cell loss at 12 months postoperatively (p>0.05). The mean UDVA improved from 0,32±0,29 to 0,41±0,33 and CDVA 0,73±0,24 to 0,74±0,26 in Snellen line at one year follow-up examination. Manifest spherical equivalent (SE) improved from -3,27±3,04 D to -2,69±2,53 D.
The mean Kmax remained stable from 49,40±6,23 D preoperatively to 49,63±6,11 D at one year follow-up. Mean corneal stromal demarcation line depth was 242,5±23,00 µm. No progression or sight threating complication was seen in any patient eye.
Conclusions:
The results reveals that accelerated corneal CXL seems to be safe and effective in halting the progression of keratoconus at one year follow-up.
