Safety, efficacy and biomechanical changes after ReLEx SMILE Xtra for borderline corneas

Abstract Details
Purpose:
To investigate the effects of lenticule extraction combined with accelerated cross linking(ReLEx SMILE Xtra) on corneal deformation parameters in a 12 month retrospective study.
Setting:
Nethradhama Eye Hospital, Bangalore
Methods:
21 eyes from 11 patients with mean age of 26.3 years, identified as moderate to high risk for ectasia(based on Randlemans ectasia risk assessment) before undergoing refractive correction with ReLEx SMILE underwent SMILE Xtra procedure using 0.25% riboflavin dye and UV-A radiation at 75 mW/cm2 for 75 seconds, delivering a total energy of 3.5mJ/cm2. Changes in corneal biomechanical properties were evaluated with Corvis Scheimpflug Technology (Corvis ST, OCULUS, Germany). Mean follow up was 12 months
Results:
12 month post, the mean spherical equivalent(SE) reduced significantly from -4.36 ± 1.79 to -0.166±0.58D (p=0.00). The mean UDVA was -0.01 LogMAR which was comparable with mean preoperative distance corrected visual acuity(DCVA)of 0.06 LogMAR (p=0.11 ). A significant reduction was observed for corneal velocity-1 (CV1) (pre=0.137m/s, 12 month=0.125m/s ), Applanation length-2 (AL 2) (pre=1.48mm, 12 month=1.23mm),Radius(Rad) (pre=6.87mm, 12 month=5.98mm)and central corneal thickness(CCT) (pre=508.7µ, 12 month=388.19µ), while pachymetry corrected IOP(IOP Pachy) showed significant increase compared to pre-operative values(pre=17.68mmHg, 12 month=23.33mmHg)(p-values <0.05 for all parameters) . No eye developed haze or progressed to ectasia at the end of mean follow-up.
Conclusions:
ReLEx SMILE combined with accelerated cross linking was safe, effective and demonstrated good biomechanical stability in 12 month follow-up.
