Purpose:
To describe an atypical unilateral presentation of VKH in a 44-year-old Filipino female.
Setting:
University of Santo Tomas Hospital, Manila, Philippines
Methods:
This is a retrospective and descriptive case report based on clinical records, laboratory and diagnostic examinations.
Results:
Vogt-Koyanagi-Harada (VKH), is an autoimmune disorder of melanocytes. Findings have been described as important in its diagnosis, one being its bilateral ocular involvement. We report an unusual case of VKH in a 44-year-old female presenting with a four month history of blurring of vision of the left eye. Best-corrected visual acuity (BCVA) was 20/20 in the right eye and 20/200 in the left eye. Slit lamp findings were normal. FA revealed features of VKH in only one eye including diffuse choroiditis, leakage, and hyperfluorescence of the optic nerve. OCT disclosed neurosensory retinal detachment. Patient improved with high dose corticosteroids.
Conclusions:
It is important for clinicians to recognize this rare clinical variant.
