Endothelial cell loss after iris-claw phakic intraocular lens (Artisan) implantation: 4 to 13,5 years of follow-up time

Abstract Details

Purpose:

To determine and establish the strength of association of the risk factors involved in corneal endothelial loss following the implantation of the ARTISAN phakic lens in a medium- to long-term follow-up.

Setting:

Centro Oftalmológico Virgilio Galvis, Floridablanca, Colombia / Universidad Autónoma de Bucaramanga, Bucaramanga, Colombia.

Methods:

We reviewed the medical records of 82 eyes of 53 patients who underwent an ARTISAN phakic intraocular lens between February 1998 and July 2015. Refractive results and corneal endothelial cell loss and related factors were analyzed.

Results:

82 eyes were included: 69 myopic (84.2%) and 13 hyperopic eyes (15.8%). The preoperative spherical equivalent in myopic eyes was -12.01 +/- 5.2 D and the postoperative spherical equivalent was -0.62 +/- 0.81D. In hyperopic eyes it was preoperatively +5.11 +/- 2.5 D and postoperative -0.39 +/- 0.89 D. Endothelial cells loss in myopic eyes was 16.04% and in hyperopic eyes was 9.24%. 3 cases finally had less than 1200 cells / mm2. Women and those over 30 years lost more endothelial cells, OR 0.29 [0.089-0.99 and OR 4.41 [ 1.04-18.71] respectively.

Conclusions:

In the medium and long term, there was an increased loss of corneal endothelial cells in an important percentage of the eyes with an iris-claw type phakic lens implanted.
