Prepupillary and retropupillary implantation of Artisan® aphakia iris-claw lens: long-term outcomes

Purpose:
To describe the demographic data, to evaluate the long-term refractive and anatomical outcomes, and to report the incidence of complications of prepupillary and retropupillary implantation of the Artisan® aphakia iris-claw intraocular lens.
Setting:
Complexo Hospitalario Universitario de Santiago, Santiago de Compostela, Spain.
Methods:
This study is a retrospective interventional case series of 101 eyes (99 patients median age 67, IQR 57-78) who underwent an Artisan® iris-claw IOL implantation at our centre between 2006 and 2016. The sample was divided into two groups: prepupillary (n=62) and retropupillary (n=38). 58% were primary implants. All of them were examined before and after surgery. The data collected included best corrected visual acuity (BVCA), anatomical changes and operative and postoperative complications. Endothelial cell count (specular microscope, CSO Ophthaltec) and development of macular edema (Cirrus HD-OCT 4000 SDT Cirrus) were also studied. Statistical analysis was performed with SPSS.
Results:
Indications for surgery were subluxation or total dislocation of IOL or lens (n=24, n=17), aphakia (n=32), trauma (n=15), Marfan (n=3), others (n=10).
There was a mean gain of 10 letters in the prepupillary group and of 15 letters in the retropupillary group. The latter result was statistically significant (p=0.047).
No statistically significant differences were found between both groups for complications (p>0.05).
Reduction of endothelial cell count was found, which was greater in the prepupillary group, although not statistically significant. Macular edema development was observed in 21.4% of prepupillary and 20% of retropupillary implants (appearance: 3 and 8 months, respectively).
Conclusions:
The study findings suggest that Artisan® IOL retropupillary implantation offers superior refractive results compared to prepupillary implantation. In addition, this series identified reduced endothelial cell loss and later development of macular edema in retropupillary implants.
