Abstract Details
Purpose:
To report the visual outcomes , safety and predictability of the iris-claw intraocular lens implantation in eyes without sufficient capsular support.
Setting:
Department of Clinical Ophthalmology, Medical University of Silesia, District Railway Hospital, Katowice, Poland
Methods:
A retrospective analysis of 104 eyes who underwent the iris fixated intraocular lens implantation in : cataract surgery (52 eyes , group 1) , post- cataract surgery aphakia (24 eyes, group II) ,keratoplasty (11 eyes , group III), pars plana vitrectomy due to the posterior chamber dislocated IOL (17 eyes, group IV). Best corrected visual acuity( BCVA) intraocular pressure (IOP) spherical equivalent (SE) were evaluated before surgery, 7 days and 6 months after surgery. The data was analyzed in Statistica 12.5 (StatSoft, Cracow, Poland) by the ANOVA Friedman and Wilcoxon tests.
Results:
The mean postoperative BCVA in comparison to preoperative BCVA improved in group I (0,15 ± 0,17 vs 0,39±0,28; p<0,001) , II (0,32±0,31 vs 0,46±0,32 ; p<0,05) , III (0,01±0,01 vs 0,1±0,02 ; p<0,001) ,in group IV was not improved (0,44±0,36 vs 0,27±0,28 ; p>0,05) . SE significantly decreased in group II. The mean postoperative IOP in comparison to preoperative IOP in all groups were not significantly changed. Post-operative complications included malignant glaucoma ( 0,96 %) , IOL subluxation caused by trauma (1,92 %) , significant IOP rise (1,92 %) , endotphathlmitis (0,96%) ,retinal detachment (1,92 %) ,macular edema (1,92 %).
Conclusions:
Iris - claw intraocular lens implantation is an effective and safe procedure in eyes without sufficient capsular support. Visual outcome depends on the primary indication.
