Purpose:
Salzmann's nodular degeneration (SND) is a rare, non-inflammatory and slowly progressive degenerative disease of the cornea. SND is characterized by the appearance of gray-blue nodular opacities that vary in number and size. Generally it is bilateral, with a female preponderance. The clinical features usually depend on the location of the nodules, but in many cases the fibrotic nodules produce an alteration in the corneal curvature causing an astigmatic refractive change leading to decrease in vision in addition to eye discomfort. we analyze the refractive change in patients with DNS before and after surgical treatment

Methods:
We analyzed 10 eyes with SND undergoing surgical treatment due to visual alteration or ocular discomfort of the patient. The size and depth of the nodules were evaluated by OCT, and their repercussion on corneal curvature through topography, aberrometry and refractometry. The surgical technique consisted of corneal epithelial delamination of the area to be treated after application of alcohol at 20%. Then nodule removal along with surrounding fibrosis was performed by traction keratectomy.
Results:
All eyes with SND that underwent surgery had an adequate and uncomplicated postoperative recovery achieving complete corneal epithelization.
The pre and postoperative comparative study (at the month of surgery) showed a significant change in corneal astigmatism with reduction of astigmatic values assessed by topography and refractometry. The study of the corneal morphology by OCT showed a change in the anterior and posterior corneal curvature causing its regularization. An improvement in vision and an attenuation of ocular discomfort were observed in all cases.
Conclusions:
The surgical removal of the nodules and the surrounding fibrosis provides a significant reduction of the corneal astigmatism caused by the SND.
