Purpose:
To analyze the corneal shape in young patients with keratoconus before and after the implantation of intracorneal ring segments (ICRS).
Setting:
FISABIO-Oftalmología Médica (FOM). Cornea and Anterior Segment Unit. Valencia, Spain.
Methods:
This was a case series of 19 eyes from 16 patients aged between 14 and 18 years old, with progressive mild to moderate keratoconus and contact lens intolerance undergoing implantation of Ferrara ICRS. Three eyes of 3 patients were excluded from the study, due to ring extrusion, caused by uncontrolled ocular rubbing habit. Femtosecond laser (IntraLase®) was used in all cases to dissect channels inside the cornea. Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), refractive error, pachymetry and keratometry were evaluated at 1, 6, 12 and 24 months of follow-up and subsequently analysed with SPSS statistic.
Results:
Mean preoperative maximum keratometric value (K max) significantly decreased from 52.1 diopters (D) preoperatively to 48.5 D at one month postoperatively. This change has remained stable after 2 years of follow-up after the surgery (p<0.05). However, there has been no subsequent statistically significant increase in UCVA and BCVA in these patients. Sphere,cylinder and pachymetry also have not experienced statistically significant differences
Conclusions:
Using the femtosecond laser for tunnel creation made the procedure easy with an accurate precise depth of implantation, especially in children. Analysis of the outcomes after Ferrara ICRS implantation showed a significant postoperative corneal flattening which remained stable up to 2 years of follow-up. Other parameters did not experienced statistically significant difference possibly due to the limited number of patients. We are currently, continually updating sample numbers.
