Purpose:
To compare the epithelium (air–tear film) and Bowman layer’s tomographies in patients with moderate to severe keratoconus.
Setting:
Prospective interventional case series in a tertiary public referral centre.
Methods:
Prospective interventional case series. Galilei dual-channel Scheimpflug combined with a Placido disc topographer was used to measure anterior corneal topography in patients having cornea collagen crosslinking for keratoconus before and after epithelial debridement. The differences in pachymetry (um), axial keratometry (K), asphericity (Q value) and astigmatism magnitude (toricity) were computed in the central 0-4, mid 4-7, and peripheral 7-10 mm corneal zones.
Results:
The study comprised 10 patients (10 eyes). The central and mid-corneal zones were thinner by 27.41 um and 32.33 um following epithelium removal. There were statistically significant differences in axial mean K of +2.34 D (p<0.001) in the central and -1.72 D (p=0.018) in the peripheral zones following epithelial debridement The change for mid-corneal zones this did not reach statistical significance (p=0.124). The astigmatism measurements differed by +3.35 D after epithelial debridement (p=0.005).
Conclusions:
In eyes with moderate to severe keratoconus, the topography of Bowman layer was significantly steeper than that of the epithelium. The epithelial layer tended to reduce the magnitude of Bowman layer’s astigmatism.
