Postsurgical results of regular and one-step higher toric intraocular lens model selection using preoperative AS-OCT data in eyes with ATR astigmatism

Abstract Details

Purpose:

To compare postsurgical results of toric intraocular lens (T-IOL) implantation in different approaches of the cylindrical power selection in eyes with against-the-rule (ATR) astigmatism.

Setting:

Ryugasaki Saiseikai Hp, Ibaraki, Japan

Methods:

On the basis of informed consent and agreement, sixty eyes of 42 subjects with ATR corneal astigmatism scheduled to undergo cataract surgery with T-IOL implantation were divided into two groups. In one group (conventional group), T-IOL model was selected according to manufacture’s T-IOL calculator, and the one-step higher model was chosen in the other (higher power group). Visual acuity (distance and near), residual astigmatism, and contrast sensitivity were measured preoperatively and at 1 day, 1 week, 1month, and 3 month postoperatively. IOL misalignments were evaluated at 3 month postoperatively. All obtained results were compared between the two groups.

Results:

Uncorrected distance visual acuity (UDVA) at 3 month postoperatively was -0.01± 0.10 (mean ± SD) logMAR in the conventional group and 0.02 ± 0.16 logMAR in the higher power group. Uncorrected near visual acuity (UNVA) was 0.70 ± 0.14 and 0.70 ± 0.17, respectively. Subjective astigmatism was 0.28 ± 0.44 and 0.23 ± 0.36 D, respectively. Contrast sensitivity (AULCSF) was 1.84 ± 0.22 and 1.91 ± 0.24, respectively. IOL misalignments were 3.7 ± 3.9 and 2.4 ± 2.0 degrees, respectively. There were no significant differences in UDVA, CDVA, subjective astigmatism, contrast sensitivity, and IOL misalignments between two groups.

Conclusions:

In eyes with ATR astigmatism, postsurgical results of one-step higher T-IOL model were equivalent to those of regular T-IOL model based on the results of manufacture’s calculator, when cylindrical power and axis were evaluated with AS-OCT preoperatively.
