Purpose:
This study describes the management of a case that developed IFS and uncontrolled OHT with Urrets-Zavalia syndrome one week after uncomplicated unilateral LASIK for myopia.

Methods:
A 41-year-old man presented with acute pain blurred vision on his right eye one week after LASIK employing mechanical microkeratome in another centre. His VA was CF and his IOP was 44 mmHg. Examination showed a non-reactive pupil with moderate mydriasis and iris transillumination. IFS was diagnosed on the slit lamp and was confirmed with OCT images showing the presence of space-occupying interface fluid. Despite full medical treatment for glaucoma including systemic acetazolamide his IOP remained uncontrolled and the optic nerve showed signs of glaucomatous cupping for which he underwent Baerveldt tube implantation.
Results:
His IOP remained under normal limits after glaucoma surgery. He also developed epithelial ingrowth that was successfully managed with flap lifting and MMC. His visual acuity improved to 6/12.
Conclusions:
IFS and Urrets-Zavalia syndrome can developed after LASIK, a non intraocular procedure, the mechanisms involved are unknown but might be related with IOP fluctuations and inflammation induced by eye suction required in LASIK.
