Early onset post-PRK ectasia managed by collagen cross-linking

Abstract Details

Purpose:

To report early onset unilateral kerectasia after bilateral PRK for low myopia in a young girl.

Setting:

Cornea and anterior segment services, Sanjivni Eye Care, Ambala, Haryana, India

Methods:

Medical records of a young lady who presented with early onset unilateral post PRK ectasia were reviewed for pre-operative topography findings, management protocol of ectasia and corneal findings during 1 year follow-up period.

Results:

21 years old lady presented with kerectasia in RE 1 month after bilateral PRK for low myopia (-2.75/-0.75@100 RE and -3.75/-1.00@80 LE). UCVA was 20/60 and 20/20 in RE and LE respectively. The ectasia in RE progressed in next one month and was managed by epi-off collagen cross linking (Dresden protocol). The preoperative topography had no suspicion of keratoconus and pachymetry was 472 and 478 microns in RE and LE respectively. The surgery was perfomed using wavelight 400Hz excimer laser (6.0 mm optical zone). The serial topography showed no sign of progression of ectasia in RE after CXL. LE topographies were normal. Final BSCVA in RE was 20/60 improving to 20/20 with rigid gas permeable lens.

Conclusions:

There are chances of ectasia even after surface ablation for low myopia that warrants a better understanding of corneal biomechanics beyond use of topography and pachymetry.
