SALK in refractive surgery-induced corneal opacity: a viable option

Abstract Details

Purpose:

To report a case of post lasik flap dehiscence with descmet folds and scarring managed with SALK (Superficial anterior lamellar keratoplasty).

Setting:

Cornea Services, Dr R.P Centre for Ophthalmic Sciences, All India Institute of Medical Sciences, New Delhi, India

Methods:

32 year-old female who underwent LASIK (Laser in situ keratomileusis) 2 years back with unaided visual acuity (VA) of 6/6 both eyes post lasik was involved in a road traffic accident and sustained blunt trauma to the Right eye 5 months back. Patient came to us with VA of 1/60 in RE was having descmet folds with scarring due to flap dehiscence at the time of accident and also having macular scar post berlin macular oedema.

Results:

SALK has been done in affected eye after doing Anterior Segment Optical Coherence Tomography (ASOCT) to assess the level of scarring and corneal topography to assess corneal parameters. Postoperative VA of 6/24 was noted. Contact lens trial was planned 4 weeks post operatively.

Conclusions:

Superficial anterior lamellar keratoplasty is a viable and effective alternative for anterior stromal scars post refractive surgery. Whereas if patient present early prompt treatment with flap lifting, debridement of epithelial ingrowth if any, reposition and sealing of the flap with fibrin glue is the main stay of management with good outcomes.
