Toric and beyond: the foundational aspects of developing an intraocular lens calculator

Abstract Details

Purpose:

To illustrate the core foundational issues in the development of a modern intraocular lens calculator.

Setting:

A research center and clinical/surgical center delivering surgical care to patients presenting with cataracts with or without significant corneal astigmatism. The EYE Center,Champaign Illinois, USA

Methods:

Fundamental optics, methods and papers described in the literature as well as clinical/surgical data were used to develop a set of principles for the development of modern intraocular lens calculators, including those providing for toric lenses. These methods were compared to methodologies of commercially available calculators.

Results:

Common features are shared by existing methodologies while they diverge in certain aspects such as the type of toricity ratio used. We identify specific areas where improvements can be made on the basis of fundamental understanding of the theory and clinical results. For example the importance of effecting a simultaneous computation of the spherical and toric components of the IOL is illustrated. Other examples are given and clinical cases of patients with significant astigmatism further demonstrate the methods of selection of their optimal lens following cataract surgery.

Conclusions:

A set of common tools in commercially available toric IOL calculators are identified and supplemented by new methodologies allowing for optimal results.
