Medium-term postoperative results of Y-fixation technique used for intrascleral fixation of multifocal IOL

Purpose:
Intrascleral haptic fixation of posterior chamber intraocular lens (IOL) has been recently reported as a new secondary IOL implantation technique. In this study, 2-year postoperative visual outcomes and complications of Y-fixation technique used for intrascleral haptic fixation of multifocal IOL were evaluated.
Setting:
Department of Ophthalmology, Juntendo University Shizuoka Hospital, Shizuoka (Japan)
Methods:
Eyes operated with Y-fixation technique (Ohta T, et al, JCRS, 2014) for intrascleral fixation of multifocal IOL from June 2012 to January 2013 were included. All patients were evaluated for postoperative uncorrected distance and near visual acuity, pre and post-operative status (slitlamp mydriatic examination for inflammation signs, IOL centration and fundus examination), intraocular pressure (IOP), complications and IOL position, using anterior segment optical coherence tomography (OCT) and wavefront sensor. A development of cystoid macular edema (CME) was also examined with OCT.
Results:
A total of 5 eyes of 4 patients (4 men, 1 woman) with a median age of 32.6 years (range 21 to 50 years) were analyzed. The median follow-up was 27.2 months (range 24 to 31 months). The preexisting ocular condition of all cases was aphakia. There were excellent results in postoperative uncorrected distance and near visual acuity of all cases, and there were no cases of significant IOL decentration and tilt, or severe complications such as CME, retinal detachment and endophthalmitis. The mean intraocular coma aberration (4 mm) was 0.05 μm.
Conclusions:
Results obtained 2 years after Y-fixation fixation technique for intrascleral fixation of multifocal IOL showed good visual outcomes without serious complications.
