Comparison of clinical outcomes after implantation of diffractive multifocal IOLs with different add powers

Abstract Details

Purpose:

To compare clinical outcomes after implantation of diffractive multifocal intraocular lens (IOL) with different add powers.

Setting:

Department of Ophthalmology, the Institute of Vision Research, Yonsei University College of Medicine, Seoul, South Korea

Methods:

This study comprised patients who underwent cataract surgery with implantation of diffractive multifocal IOL with a +2.75D additional power of (Group 1, 23 eyes), with +3.25D (Group 2, 21 eyes) or with +4.00D (Group 3, 21 eyes). Postoperative evaluations were performed at one month and three months after cataract surgery. Uncorrected distance visual acuity (UDVA), manifest refraction (MR) values and uncorrected near visual acuity (UNVA) were checked at 33cm, 40cm and 50cm at one and three months after surgery. At 3-month follow-up, internal aberrations and contrast sensitivity was evaluated and patients were given a questionnaire regarding the satisfaction.

Results:

There were no significant differences of UDVA and MR values among the three groups. UNVA at 33cm was better in group 3 (p=0.031, 0.032) and UNVA for 50cm was significantly better with group 1 than other two groups at 3 months postoperatively (p=0.035) No significant differences of internal optic aberrations and contrast sensitivity were demonstrated among three group. Overall satisfaction after the surgery was the worst in group 3 (p=0.004). Presence of halo and glare was more frequent in group 3 without statistically significance. Complete spectacle independence was higher as 87.0% in group 1 than other groups (p=0.08).

Conclusions:

Implantation of diffractive multifocal IOLs with lower add powers provided good visual acuity for longer working distance and excellent distance visual acuity. Patients showed higher satisfaction and spectacle independence with lower visual symptoms.
