Contrast sensitivity and defocus curve of multifocal intraocular lenses: comparative study

Purpose:
To compare visual quality, using contrast sensivity (CS) and visual functioning (defocus curve) after implantation of three types of Multifocal Intraocular Lenses (IOL): Tecnis® (+3,25D), ReSTOR® (+3,00D) and AT LISA trifocal® (+3,33D near add e 1,66D intermediate).

Methods:
We evaluated 85 eyes from 45 patients with ages between 45 and 70 years old. There were 2 control groups: one with a monofocal IOL and another without IOL implantation, both within the same age range. Defocus curves from +2,5D to -4,5D were obtained in binocular vision, with distance correction. CS was tested using patterns of vertical sine-wave gratings with 3 distinct inclinations and 6 different spatial frequencies (from 0,75 to 18 c/d) in photopic and mesopic conditions.
Results:
No statistically significant differences were observed between visual behaviour of three multifocal IOLs for the dioptric powers between +1,5D to -1,5D, although AT LISA trifocal® showed better visual acuity for the -1,00 and -1,5 D adds. The defocus curve of the monofocal IOL shows a significant decrease of visual function for the -1,5 D or more of near add, compared to any kind of multifocal IOL. The ReSTOR IOL shows the best outcome for the -4,00 and -4,50 D add. The AT LISA trifocal® IOL showed statistically superior CS values than the Tecnis® and ReSTOR® IOL in low spatial frequencies (0,75- 3 c/d) in both light conditions, and similiar results as the monofocal IOL in both mesopic and photopic conditions.
Conclusions:
The AT LISA trifocal® IOL provides better visual acuity at intermediate distances than the classic bifocal IOLs and shows the smallest contrast sensivity loss of all multifocal IOLs studied, providing similar results as a monofocal IOL for low spacial frequencies.
